How do you tell someone you really like them without making a fool of yourself?

I really like an older guy, and I think he likes me too as the eye contact is amazing.
Problem is I've really fallen for him but am really struggling to tell him!
I just freak out everytime Im about to do it!!!
Any tips on how I should do it with minimal embarrasment?
